NOTE: The “Applicant” is the person who will be receiving and using the NJPA Press Identification Card, and in whose name the Card will be issued, if approved.
Name, title and contact information of the person submitting application on behalf of Applicant(s):
Provide a list of names, in alphabetical order by last name, of all applicants for whom NJPA Press Identification Cards are being requested and provide the job title for each
Applicant’s Title must be one of the following: Publisher, Editor, Reporter or Photographer
Next Calendar Year 2025 (valid, if issued this year, from January 1 of the next calendar year through December 31 of the next calendar year and, if issued next calendar year, valid from the date issued through December 31 of that calendar year)**
** The time periods for which the NJPA Press Identification Cards are valid apply only:
Applicant has been advised and has acknowledged an understanding that, if issued an NJPA Press Identification Card, it is intended for Applicant’s use only and Applicant will not permit it to be used by any other person.
Applicant has also been advised of and understands the Criteria for issuance of an NJPA Press Identification Card and represents that, to the best of Applicant’s knowledge, information and belief, Applicant satisfies those Criteria. Applicant has further represented that, if issued an NJPA Press Identification Card, Applicant will notify NJPA at such time that Applicant no longer satisfies the Criteria and that, at such time that Applicant no longer satisfies the Criteria, Applicant will not use the NJPA Press Identification Card for any purpose.
